Understanding the Stages of Retinal Detachment

The retina is a thin layer of tissue at the back of the eye. It works like a camera, capturing light and sending visual signals to the brain. How a Retinal Detachment May Occur

A detachment happens when the retina separates from the tissue that supports it. Once this layer lifts away, the affected cells lose the oxygen and nutrients they need to work well. The longer the retina stays detached, the more it affects an individual’s vision. Many detachments start with a small tear or hole in the retina. As people age, the gel inside the eye can shrink and tug on the retina, which sometimes leads to a tear. Fluid can then slip through the opening and push the retina away from the wall of the eye.

Other health factors can raise the risk of developing retinal detachment, such as diabetes or a traumatic eye injury. Risk factors for this condition include a hard blow to the eye, very high nearsightedness, past eye surgery, and a family history of detachment. People with diabetes also face a higher risk of developing retinal issues due to changes in the blood vessels of their eye(s). There are three main types of retinal detachment: 

  • Rhegmatogenous Detachment: This type is the most common, and it begins with a tear that allows fluid to build up behind the retina.
  • Exudative Detachment: This type happens when fluid collects under the retina without a tear, often due to swelling or disease.
  • Tractional Detachment: This type occurs when scar tissue forms on the retina, pulling it away from the back of the eye.

How This Condition Impacts Vision

Symptoms of retinal damage often appear in stages, and in the early stage, individuals may notice new floaters in their field of vision. Floaters look like small specks or threads drifting across the visual field. When someone has a detached retina, they might also see brief flashes of light, like quick sparks at the edge of their vision. As this condition progresses, a shadow or gray curtain may spread across part of their vision. This curtain usually starts on one side and grows over time. In the later stage of a detachment, central vision can blur or fade as more of the retina pulls away.

How This Condition Is Treated

Signs of retinal issues can develop quickly, so getting timely medical attention is key to preserving vision and eye health. Treatment depends on the type of detachment and how far it has progressed. For a small tear, an eye doctor may use laser therapy to seal the area before fluid builds up. The laser causes scar tissue, which keeps the retina in place. For a fuller detachment, surgery is often used.

One surgical option is a pneumatic retinopexy, which places a small gas bubble in the eye to push the retina back into place. Another method, called a scleral buckle, adds a tiny band around the outside of the eye to support the wall. A vitrectomy approach removes the buildup of fluid inside the eye, replacing it with gas or air. The appropriate treatment option depends on an individual’s eye health and the advice of their doctor.
