An N0 status indicates no lymph node involvement. N1 through N3 classifications reflect increasing levels of lymph node involvement; these values depend on the number and location of affected nodes. Since lymph nodes act as filters in the immune system, surgeons may remove them during surgery, and pathologists examine them under a microscope to assess spread. This evaluation helps determine how breast cancer has progressed; it supports decisions about additional treatment when lymph nodes are affected.

Determining Distant Spread

The letter M stands for metastasis in breast cancer staging. Metastasis describes cancer spreading to distant organs, and this represents advanced disease. If breast cancer reaches organs such as bones or lungs, doctors classify it as stage IV, and they focus treatment on controlling the spread of the disease. An M0 classification indicates no distant spread. Doctors may order scans or blood tests, and these tools help detect whether cancer has spread beyond the breast and lymph nodes. Because metastatic disease requires systemic therapy, oncologists may use chemotherapy or targeted treatments, and patients undergo follow-up imaging to monitor response.
